How this works

The State of Missouri has approved your care plan. That plan lists the tasks you will get help with, and how many days each week you are approved for each one.

Some tasks are approved for every day, 7 days a week. You do not need to pick days for those. They will be scheduled daily.

Other tasks are approved for a set number of days each week, such as 1, 2, 3, 4 or 5 days. For those, you choose which days of the week you would like them done.

The days you choose here go onto your schedule. Your attendant follows that schedule, so they know which tasks to do on which days. If you want to change your days later, call the office and we will update it.

Section 1: Tasks scheduled every day

The State of Missouri requires these tasks to be scheduled every day, 7 days per week. You do not need to choose days for them.

Section 2: Tasks where you choose the days

For each task below, the state approved a set number of days per week. You choose which days those are. The form will only let you pick as many days as you were approved for.

Section 3: Verification
"I understand that my care plan is created and approved by the State of Missouri. I do not select my tasks or how many days I receive them. I only choose the days for tasks that are scheduled less than 7 days per week. I have reviewed my care plan and selected my preferences."
